Welcome New Patients!

Congratulations on making a commitment to your own health and well being!  I am looking forward to beginning our work together, and will be honored to serve as a guide and partner in your journey toward your ideal state of health and balance. 

Required paperwork for new patients is extensive.  I highly recommend downloading all 5 of the forms below and completing them before arriving for your first visit.

   *The form above is very long. If you decide to print this as a double sided document to save paper PLEASE print the back side (even numbered pages) UPSIDE DOWN. (this makes it much easier to read once it is in your chart).

   If you are coming in to address issues of fatigue, chronic aches and pains, mental fog, menstrual issues, headaches, or thyroid issues, please also consider downloading and completing this temperature log before your first visit.

As a naturopathic physician, acupuncturist, and massage therapist (multi-credentialed provider), I am a bit of an insurance anomaly.  As far as the insurance companies are concerned I am three different practitioners.  Because of this, there are occasionally insurance plans where I am a preferred provider under only one of my licenses.  In addition every insurance policy is unique in its benefit details and limitations.  Many patients find it both helpful and educational to clarify their insurance coverage using our Insurance Information form as a guideline for asking clarifying questions about their policy.  Make sure you confirm that I am both a naturopathic physician and an acupuncturist on your preferred provider list.  If I am not on your list, you may have coverage for “out of network” providers.
